Diabetes Care Pharmacist

PacMed believes in a team-based approach when managing diabetes. This could include your provider, a dietitian, and a clinical pharmacist to help patients achieve better health. The comprehensive care model allows our team to educate patients on lifestyle modifications, specific diabetes medications, and other facets of diabetes management tailored to each patient.

Our pharmacist works collaboratively with your care team to help provide medication education and ensure patients understand how medications fit into their daily life. PacMed has a dedicated pharmacist for patients receiving diabetes care. This includes discussing medications, education on how the they may fit into your individual lifestyle, how to take your medicine, what reactions may occur, and answering questions.
